HMO Plans
HMO plans are designed for individuals and families who want predictable costs and coordinated care. These plans focus on in-network providers and typically require you to choose a primary care physician who helps manage your healthcare needs.
They’re a strong option if you’re comfortable staying within a network and want lower monthly premiums with simpler billing.
This plan may be a good fit if you:
- Want lower monthly premiums and predictable out-of-pocket costs
- Are comfortable using a primary care physician for referrals
- Don’t expect to need frequent out-of-network care.
EPO Plans
EPO plans offer a balance between cost and flexibility by giving you access to a network of providers without requiring a primary care physician referral. You’ll typically need to stay in-network for coverage (except emergencies), but you can often see specialists directly.
They’re a strong option if you want lower premiums than many PPO plans and like the convenience of skipping referral requirements.
This plan may be a good fit if you:
- Want to see specialists without needing referrals
- Prefer lower monthly premiums but can stay in-network
- Don’t expect to need frequent out-of-network care.
PPO Plans
PPO plans provide the most flexibility when choosing providers. You can typically see specialists without referrals and you may have partial coverage for out-of-network care, depending on the plan details.
They’re a great option if you value freedom of choice, travel often, or want broader provider access—even if the monthly premium is higher.
This plan may be a good fit if you:
- Want maximum flexibility in choosing doctors
- Travel often or want access outside your local network
- Prefer out-of-network coverage options (even if it costs more)
POS Plans
POS plans combine features of HMO and PPO coverage. You’ll usually choose a primary care physician, but you may have the option to go out-of-network for certain services with a referral and higher out-of-pocket costs.
They can be a good middle-ground if you want structured care coordination but still want occasional out-of-network flexibility.
This plan may be a good fit if you:
- Like having a primary doctor coordinate care
- Want some out-of-network options when needed
- Prefer a middle-ground between cost and flexibility
