Health insurance plans in Miami-Dade County, Florida (2026)

I pulled every 2026 marketplace plan sold in Miami-Dade County, Florida out of the official CMS plan file. There are 189 plans from nine carriers, and a 40 year old in Miami-Dade pays $684 a month at full price for the benchmark silver plan. That puts Miami-Dade in the middle of the 20 Florida counties I track, with 12 cheaper and 7 pricier. Across all 79 counties and parishes I track in 24 states, 22 have a higher benchmark. Cigna also sells in Miami-Dade now and is leaving for 2027, so some of your neighbors have to switch plans this fall. Below are the carriers, the plan counts, the full prices before subsidy, and what I think the numbers mean if you live in Miami, Hialeah, Homestead, Miami Beach or anywhere else in the county.

Who sells marketplace plans in Miami-Dade County

Nine companies sell in Miami-Dade, against ten in the busiest Florida county I track and four in the thinnest. The biggest lineup in Miami-Dade belongs to Florida Blue with 65 plans, about 34 percent of everything on offer.

Leaving Miami-Dade County for 2027

Cigna sells 13 of the 189 plans in Miami-Dade County this year. Cigna is leaving the ACA individual market in every state where it sells, on and off exchange, for 2027. After that exit, eight carriers are left from the 2026 Miami-Dade lineup: Florida Blue, Ambetter Health, Molina Healthcare, UnitedHealthcare, Oscar, Wellpoint, AvMed and AmeriHealth Caritas Next. If you are enrolled with Cigna in Miami-Dade, you will get a notice this fall. If you do nothing you are moved to a replacement plan picked on price and metal level, and nothing in that process checks your doctors or prescriptions.

What it costs in Miami-Dade County before any subsidy

These are the full monthly prices, before any premium tax credit, for the cheapest Miami-Dade plan at each metal level. I show the sticker price because it is what your subsidy is subtracted from, and most people who enroll in Miami-Dade pay less, often a lot less.

Monthly premium, full priceAge 21Age 40Age 60Couple, both 40Couple 40 + 2 kids
Lowest bronze$393$502$1,065$1,003$1,604
Lowest silver$534$682$1,448$1,364$2,180

Lowest gold$488$624$1,325$1,248$1,995
Lowest platinum$1,298$1,659$3,523$3,318$5,304

Benchmark silver is the second lowest of the 72 silver premiums listed for Miami-Dade. The last column is two adults aged 40 plus two children.

What these numbers mean if you live in Miami-Dade County

The gold row is the surprise in Miami-Dade County. The cheapest gold plan for a 40 year old, $624 a month, is $58 cheaper than the cheapest silver plan at $682. That happens because Florida carriers load the cost of the extra silver discounts into silver premiums. If your income is too high for those discounts, silver is the worst value on the Miami-Dade shelf, and a gold deductible here tops out at $3,000.

In Miami-Dade County the benchmark is $183 above the cheapest bronze at age 40, and 37 of the other 78 counties and parishes I track have a wider one. That gap decides whether bronze can be free, because your subsidy is the $684 benchmark minus what the government expects you to pay from your income. A 40 year old in Miami-Dade whose expected share is $183 a month or less would pay nothing for the cheapest bronze. At 60 the Miami-Dade gap grows to $388 and at 21 it shrinks to $143, which is why older enrollees are the ones most likely to find a $0 plan.

Prices barely differ between Miami-Dade and nearby Broward County. The benchmark differs by $3 a month at 40 and the cheapest bronze by $1. 22 Health sells in Broward County and not in Miami-Dade.

By network type the 189 Miami-Dade plans break down as 138 HMO, 27 EPO, 14 PPO and 10 POS. Only 14 are PPOs, and every one of them comes from Florida Blue. If seeing specialists without a referral matters to you, that narrows the Miami-Dade shelf to one company. 28 plans in Miami-Dade have a $0 medical deductible (17 expanded bronze, 6 platinum and 5 gold). The 17 bronze ones among them still have out of pocket maximums of $9,900 to $10,600, so a $0 deductible does not mean a small bill.

Every Miami-Dade price on this page is before subsidies, and most enrollees pay less. For a 40 year old in Miami-Dade the premium tax credit is measured against the $684 benchmark. Estimate your own subsidy, or send me your ZIP and I will run the Miami-Dade numbers for you.
